Cold Therapy vs. Heat Therapy: What Does the Science Really Say?

From cold plunges and ice baths to saunas and heating pads, temperature therapy has become one of the hottest topics in health and wellness. Social media is filled with bold claims about boosting metabolism, improving mental health, reducing inflammation, and 'resetting' your nervous system—but what does the science actually say? As a chiropractor, I believe the best health decisions are made by combining clinical experience with high-quality research. While both heat and cold have their place in recovery, there isn't a one-size-fits-all approach. Understanding when—and why—to use each can help you recover more effectively and support your long-term health.
Understanding Cold Therapy
Cold therapy, also known as cryotherapy, works by lowering tissue temperature. This temporarily reduces blood flow, slows nerve conduction, and decreases pain perception. Research consistently shows that cold water immersion can reduce delayed onset muscle soreness (DOMS) after intense exercise, helping many athletes feel more recovered. However, current evidence does not consistently show faster tissue healing or improved strength recovery. Cold therapy is a useful recovery tool—not a miracle cure.
The Benefits of Heat Therapy
Heat increases blood flow, improves tissue flexibility, and relaxes muscles. It is often most helpful for chronic neck and back tension, tight hips, muscle stiffness, joint discomfort, and preparing the body for stretching or exercise.
What Is Contrast Therapy?
Contrast therapy alternates between heat and cold, such as moving between a sauna and a cold plunge. Research suggests it may reduce post-workout soreness, improve perceived recovery, and help athletes return to training feeling refreshed, although more research is needed to determine whether it accelerates tissue healing.
Are Cold Plunges Different for Women?
Current scientific evidence does not show that healthy women should avoid cold plunges simply because they are women. Women may respond differently to cold due to differences in body composition and hormonal fluctuations throughout the menstrual cycle, but 'different' does not mean 'harmful.' More female-specific research is still needed.
Cold Therapy and Your Nervous System
A common misconception is that cold plunges immediately calm the nervous system. In reality, cold exposure is a controlled stressor that temporarily activates the sympathetic ('fight-or-flight') nervous system and increases adrenaline, noradrenaline, and cortisol. For healthy individuals, this short-term stress may lead to beneficial adaptations over time. However, people recovering from illness, surgery, childbirth, chronic stress, or burnout may benefit from using cold exposure more cautiously.
Women's Health: Pregnancy & Postpartum
During pregnancy, gentle localized heat may help relieve muscle tension, but prolonged exposure to high temperatures such as hot tubs and very hot saunas is generally discouraged. Cold packs can help with localized swelling and discomfort. Postpartum recovery is highly individual. While cold therapy can reduce soreness and swelling, full-body cold plunges also add physiological stress. Gentle movement, chiropractic care, mobility exercises, hydration, nutrition, and sleep remain the foundation of recovery, with cold exposure introduced gradually if it leaves you feeling energized rather than depleted.
